ADMISSION PROCEDURES AND REQUIREMENTS

   The applicant first takes the Admission Placement examination.
   (FOR FRESHMEN & TRANSFEREES only)

   Requirements and Procedures:

  • Pay the testing fee (120.00) at the treasurer's office. Present the official receipt to the testing center.
  • Submit (2) two recent 1½ x 1½ pictures with name tag (blue background)
  • Examinee must fill up the application for college entrance examination form personally at the testing center.  
  • After filling up the form, examinee will be scheduled for testing.
  • Examination will be issued.
  • No applicant will be given the said examination twice.
  1. If the applicant passes the examination he then fills up the application for admission form. To be attached to the said form are the following requirements:


    FRESHMEN:

  • Original High School Report Card/Form 138
  • Admissions Form (Blue form)
  • Certificate of Good Moral Character  (ADNU form)
  • Birth Certificate-Original or authenticated copy from the NSO/ Local Civil Registrar
  • Admissions slip (ADNU form)
  • Admissions Recommendation Form (ADNU form)
  • 4 copies 1.5”x1.5” size picture with name tag (blue background)
  • Long white folder

 

   TRANSFEREES:

  • 2 original/authenticated copies of Transcript of Records/List of subjects taken with course description duly certified by the Registrar
  • Honorable Dismissal/Transfer Credential
  • Admissions Form (Green form)
  • Certificate of Good Moral Character (Adnu form)
  • NSO copy or Local Civil Registrar authenticated copy of Birth Certificate
  • Admissions slip (Adnu from)
  • Admissions Recommendation Form (Adnu form)
  • 4 copies 1.5”x1.5” size picture with name tag (blue background)
  • Long white folder

 

   DEGREE-HOLDER:

  • 2 original/authenticated copies of Transcript of Records w/ Special Order No.
  • Original copy of Certificate of Graduation
  • Honorable Dismissal/Transfer Credential
  • Admissions Form (Pink form)
  • Admissions slip (ADNU form)
  • Admissions Recommendation Form (ADNU form)
  • 4 copies 1.5”x1.5” size picture with name tag (blue background)
  • Admissions Fee Official Receipt
  • Long white folder

 

   CROSS-ENROLLEE:

  • Original and 2 photocopies of Permit to Cross-Enroll
  • Admissions Form (Pink form)
  • Certificate of Good Moral Character (ADNU form)
  • Admissions slip (ADNU form)
  • Admissions Recommendation Form (ADNU form)
  • 4 copies 1.5”x1.5” size picture with name tag (blue background)
  • Admissions Fee Official Receipt
  • Long white folder

 

   NON-CREDIT:

  • Original Transcript of Records if transferee or degree-holder; Certificate of Employment if working
  • NSO copy or Local Civil Registrar authenticated copy of Birth Certificate
  • Certificate of Good Moral Character (ADNU form)
  • Admissions Form (Pink form)
  • Admissions slip (ADNU form)
  • Admissions Recommendation Form (ADNU form)
  • Admissions Fee Official Receipt
  • 4 copies 1.5”x1.5” size picture with name tag (blue background)

   RE-ADMISSION:

  • Admissions Form (Yellow form)
  • Admissions slip (ADNU form)
  • Admissions Recommendation Form (ADNU form)
  • 4 copies 1.5”x1.5” size picture with name tag (blue background)

*Note:
Students stop for 2 years up should take the entrance exam and please see to it that you have been cleared of your back accounts at the Treasurer’s Office and any violations at the OSA…
